Description of Responsibilities: |
| Primary responsibilities include
providing the highest quality of effective, proactive, and
professional services to members and clients in all aspects of Case
Management. Case Managers serve as a point of contact for providers
caring for patients who have entered the Avidyn Health Case
Management program. Enabling and supporting responsibilities include
all activities associated with identifying potential appropriate
chronic and catastrophic cases requiring Case Management,
communicating with and obtaining approval of client/administrator
and patient/family, performing all functions associated with Case
Management (including but not limited to arranging appropriate
alternative care settings, coordinating with the hospital discharge
planning, negotiating discounts and cost effective/high quality
services with vendors), reporting activities and case summaries to
clients, and following and recording billing for professional
services rendered. |

Awards Received: |
| Elaine writes, The most important
reward I receive is when one of my patients or family members
expresses appreciation for my case management interventions. Any
other awards are just pieces of paper tucked away in my
miscellaneous file. |

None really. My family is the most important to
me. |

Exceptional Accomplishment: |
|
When
Rose or Jessica, granddaughters who are 3 and 2 years old,
spontaneously tell me,
I love you. That is what life is all
about. |

Hobbies, Special Interests: |
| I love to make quilts and hand-quilt
them just for fun, even if it does take a few years to complete. It
is a very creative experience and process. I have yet to make a
quilt that ends as it began. I think this is a reflection of how I
feel about life. It is ever changing and you must be able to change
to survive and find happiness. |

Experience: |
| Elaine has previous experience in ICU,
CCU, ER, Director of an LVN school for over 8 years, and psychiatric
nursing for long-term young adult schizophrenics with a mean age of
20. |

What does Rehabilitation Nursing mean to you? |
| It is quite simple. Helping others to
obtain their highest potential in all areas of their life. |

Anything else that you would like to share? |
| I love to discuss complex case
management cases and attempt to find solutions to what seems to be
overwhelming issues. Call me, I always have an opinion. |
